OneOncology partners with Unum to provide supplemental coverage for health care costs associated with unexpected accidents and illnesses.

This voluntary benefit is available to provide you a lump sum dollar payment that would be made directly to you (not to hospitals or doctors) in the event that you or your dependents experience a critical health condition such as heart attack, stroke, major organ transplant, permanent paralysis, end-stage renal failure or coronary artery bypass surgery.

You can elect a lump sum benefit payment up to $30,000 that would be paid to you upon the diagnosis of a covered condition. If you are enrolled in this benefit, you can also elect a benefit for you spouse (up to $20,000) or your child (up to $5,000). A pre-existing limitation does apply, which means if you have received treatment in the past 12 months for one of these conditions, benefits are not available until you’ve been continuously enrolled in this plan for 12 months or more.

Accident

This voluntary benefit is available to provide you a fixed dollar payment based on the type of injury you sustain and its severity. Benefits are payable for things like ambulance transport or injuries such as limb fractures, burns, lacerations and dislocations. This plan is very popular with employees that have children.

For example, depending on the severity of a broken arm, this plan could provide a benefit payment that ranges from $600 to $1,200 while a small laceration benefit payment is $100.

Both of these voluntary plans include a health screening benefit, which means that $50 is paid per year for each covered person who completes at least one wellness visit, health screening test or preventive care service. Examples include, but are not limited to, routine gynecological exams, general health exams, mammography, and certain blood tests.
